Our New Partners:ACO Essential CareThe Luling Community Dental Center is made possible through a special gift from the St David’ Foundation Austin Texas.We are proud to announce that CHCSCT has added Victoria Community Health Center located at 2805 N. Navarro St Suite 320. Please call 361-576-2110 to make medical appointments, mental health or women’s health services. We have now added Victoria Dental Clinic also at 2805 N. Navarro, Suite 102 for appointments please call 361-582-4440 for dental appointments

Medical ClinicWe offer full range of medical services, including but not limited to: pediatrics, maternal child health, well child check ups, immunizations, diabetic and hypertension care and follow-up. Our staff are able to assist the patients from birth to geriatrics. Dental ClinicWe have a full service dental clinic at each site and offer preventative and maintenance care. Children receive sealant and fluoride treatment as well as education on how to care for their teeth. Women's Health / WIC ProgramThe Women's Health Center offers all aspects of well female and male reproductive care. A variety of birth control methods, Breast and Cervical Cancer screening, STDs and family planning is available. PediatricsWe offer pediatrics at each of our sites and have a Pediatrician at our Gonzales and Luling clinics. All childhood immunizations are offered through the Texas Vaccine for Children Program. Behavioral HealthAt each site Behavioral Health is offered throughout all life cycles. (Children through adulthood) PharmacyWe offer a Class D Pharmacy with limited amounts of drugs and birth control. We have a Patient Assistance Program at each of our sites to assist patients to receive their medications with a greatly reduced rate or free. LaboratoryWe offer in house labs and phlebotomy services for any testing that needs to be sent to an outside lab. Video Introduction to Medical Home
